I understand that any testimony or feedback I provide may be used to publicize and promote Haven Services. I hereby grant Haven Services, its representatives, and its employees the irrevocable right to use my:

Photograph or likeness (if provided)
Brief biographical information
Written or verbal testimonial
I authorize Haven Services to utilize these materials across various marketing initiatives and mediums. This includes, but is not limited to:
Publicity & Advertising: Printed brochures, flyers, and reports.
Digital Content: Website content, social media platforms, and email marketing.
Illustrative Purposes: Educational or promotional presentations.
I authorize Haven Services to copyright, use, and publish these materials in both print and electronic formats. I understand that Haven Services retains ownership of the finished product in which my testimonial or likeness appears.
I agree that I will make no monetary or other claim against Haven Services for the use of my name, photograph, biographical information, or testimonial. I waive any right to inspect or approve the finished product or the advertising copy that may be used in connection with this material.
